The PhilHealth online portal is an easy and convenient way for members to check and track their monthly contribution records without having to physically visit a local PhilHealth office. Access to the member portal lets you manage your online account and browse PhilHealth’s services.

1) Visit the PhilHealth website.

Choose “Online Services,” and then click on the “Register/Login” link under the Member Portal category. Click on “Create Account,” found on the lower right portion of the page.

Next, fill out the online registration form. Enter your PhilHealth ID number, as well as your complete name and other required information.

2) Fill login information

This section is where you shall create your online account password with the following requirements:

  • Passwords must have a minimum of 8 characters and a maximum of 32 characters.
    The password must contain at least one number between 0 and 9.
  • It must contain at least one uppercase letter (A–Z) and at least one lowercase letter (a–z).
    The password must contain at least one special character (` ~ ! @ # $ % ^ & * ( ) – _ = + [ ] { } | ;'” < >,.. /?).

3) Prove you are not a bot with the Captcha

Fulfill the captcha requirements by entering numbers into the text box provided or by solving a puzzle. In case the first one does not work or is too hard, you can click the refresh button to generate a new one. Once you pass the Captcha, click the “Create Account” button.

4) Wait for the confirmation message

Once you have clicked the “Create account” button, this message will appear:

“To complete your account creation, we have sent a link to <email address provided>. Please check your email to activate your account.”

5) Proceed to account activation

Check the email address you provided upon registration and open the mail from PhilHealth. Next, click on the activation link provided. You will then be redirected to a page stating that your account has been successfully activated.

You can choose to close the page or click the button to return to the login page. Use your PhilHealth Identification Number (PIN) as your login and type the password you created.

The PhilHealth Online Registration portal lets members register, manage, and keep track of their contributions online. This system is called eRAS, or electronic registration and amendment system.

The member portal is a convenient option for members to pay their premium contributions online, update their personal information, and submit reimbursement claims.

Moreover, the PhilHealth member portal provides pertinent information such as coverage, eligibility, application requirements, and other applicable benefits.

With this secure portal, members can access the website from any device, regardless of their physical location.

Obtaining a copy of the MDR

If you wish to get a copy of your MDR, you can send an email to ofp@philhealth.gov.ph and request an electronic copy of the document.

When sending your request, make sure to include the following information:

  • Complete name (last name, first name, middle name format)
  • Birth date
  • Present or permanent address

You will receive an automated email response with a copy of your official member data record.

How do I obtain a PhilHealth member registration form?

The PhilHealth Member Registration Form (PMRF) can be obtained from any PhilHealth office or the PhilHealth website.

This form needs to be filled out and duly signed by the member in preparation for online registration procedures.

The PMRF has written instructions to serve as a guide to help you accomplish the mandatory fields on the form.

Once completed, submit PMRF, together with other requirements, attachments, and supporting documents, to any PhilHealth branch office.

You will be assigned a PhilHealth Identification Number (PIN).

Your PhilHealth ID card and a copy of your MDR will be issued accordingly thereafter.

Requirements and necessary documents for PhilHealth registration

  1. Employed Members
    • Duly accomplished PhilHealth Member Registration Form (PMRF).
    • Two (2) latest 1×1 ID pictures.
    • Photocopy of one valid ID (e.g., passport, driver’s license, SSS ID, etc.).
    • For first-time registrants: Birth Certificate or any proof of birth.
    • For updating records: Certificate of Employment.
  2. Self-Employed Members
    • Duly accomplished PMRF.
    • Two (2) latest 1×1 ID pictures.
    • Photocopy of one valid ID.
    • Proof of income (e.g., latest Income Tax Return, financial statements, etc.).
  3. Indigent Members:
    • Duly accomplished PMRF.
    • Certification from the DSWD or the local social welfare office confirming indigent status.
  4. OFWs:
    • Duly accomplished PMRF.
    • Two (2) latest 1×1 ID pictures.
    • Photocopy of one valid ID.
    • Valid Overseas Employment Certificate (OEC) or proof of overseas employment.
  5. Senior Citizens:
    • Duly accomplished PMRF.
    • Two (2) latest 1×1 ID pictures.
    • Photocopy of Senior Citizen’s ID or any valid ID showing the date of birth.
    • Certification from the Office for Senior Citizens Affairs (OSCA), if applicable.
